This Privacy Policy explains how Bridgetown Dental (“we,” “us,” “our”), 8907 North Lombard St., Portland, OR 97203, collects, uses and protects information through bridgetowndental.com, our online forms and self-assessments, phone calls and text messages, and our online advertising.
Health information we create or receive while caring for you as a patient is also protected by HIPAA and is covered by our Notice of Privacy Practices, described below. When you visit our website, certain technical information may be collected automatically, such as browser type, device information, pages visited, referring pages, and general information about how the website is used. This information may be used to maintain the website, understand how visitors use it, improve performance, and improve the patient experience.
If you choose to provide information through a website form, email, scheduling request, or other online feature, we may use that information to respond to your request, communicate with you, or provide the services you requested.
Please avoid sending highly sensitive medical, financial, or personal information through ordinary email or website fields unless the feature is specifically designed and secured for that purpose.
When you request an appointment, complete one of our self-assessments, or contact us, we may collect your name, email address and phone number, the reason for your visit, your preferred day or time to be contacted, whether you are a new or existing patient, your self-assessment answers and result, and anything you write in a message field.
Our forms send this information to our office by email, and some forms also keep a copy on our website so our team can follow up. Self-assessment answers can describe symptoms, so please share only what you are comfortable sending online. You are always welcome to call us instead.
We use the information we collect to respond to your requests, schedule and confirm appointments, send reminders, answer your questions, provide care, keep our website working and secure, block spam, understand which pages and ads help people find us, and meet our legal and professional obligations.
We do not sell your personal information.
Our website may use cookies and similar technologies that help the website function, measure performance, understand how visitors use the site, and improve the user experience.
Because Bridgetown Dental is a healthcare practice, we take particular care in evaluating analytics, advertising, and other third-party technologies used on our website.
These technologies may include Google Analytics and Google Tag Manager, which measure visits and how pages are used; Google reCAPTCHA, which helps protect our forms from spam; call tracking, which can show a different phone number on the website so we know which pages or ads lead to calls; and YouTube videos, which we embed in privacy-enhanced mode so YouTube does not set cookies until you press play.
You can block or delete cookies in your browser settings, though some parts of the site may not work as well without them. You can opt out of Google Analytics with Google’s browser add-on at tools.google.com/dlpage/gaoptout, and manage the ads Google shows you at myadcenter.google.com.
We advertise our services online, for example through Google Ads. When you click one of our ads, the ad platform may let us know that your visit came from an ad, and we use that to see which ads lead to calls or appointment requests. Ad platforms may also use cookies to show our ads to people who have visited our website.
We do not send the contents of your form submissions, self-assessment answers or other health information to advertising platforms.
If you opt in to text messages, we may send appointment reminders, promotional offers and other occasional messages from our office. Opting in is voluntary and is not a condition of receiving care. Message frequency varies, and standard message and data rates may apply.
Reply STOP at any time to stop receiving texts, or HELP for help. Your mobile number and text-message opt-in will only be used to communicate with our office and will not be shared with third parties for their marketing purposes.
Bridgetown Dental maintains the privacy and security of protected health information in accordance with applicable federal and state requirements.
Our Notice of Privacy Practices explains how health information about you may be used or disclosed, your rights regarding that information, and our responsibilities for protecting it.
You may request Bridgetown Dental’s current Notice of Privacy Practices from our office. The website will also provide access to the approved notice when the final document is posted.
Depending on applicable law and the circumstances, your rights may include the ability to request access to your health information, request corrections, request confidential communications, ask for certain restrictions on uses or disclosures, request information about certain disclosures, obtain a copy of our Notice of Privacy Practices, and raise a concern regarding the privacy of your health information.
Our complete Notice of Privacy Practices explains these rights in greater detail.
We share information only as needed to run our practice and this website. That includes companies that work on our behalf, such as website hosting, email delivery, form processing, analytics, call tracking, appointment reminders, payments and membership enrollment. They may use the information only to provide those services to us.
We may also share information when the law requires it, to protect the rights, safety or property of our patients, our team or others, or as part of a sale or transfer of the practice, in which case the information stays protected by this policy and by HIPAA.
Our website uses a secure (HTTPS) connection, and access to the information you send us is limited to the team members who need it. No website or email system is completely secure, which is why we ask you not to send detailed health or financial information through ordinary forms or email.
We keep website and form information only as long as we need it for the purposes in this policy. Patient records are kept for the periods required by law.
You can ask us to stop sending you text messages or emails at any time. You can also ask us what website information we have about you, or ask us to correct or delete it, by calling or emailing our office. We may need to confirm your identity first. Some information, such as patient records, must be kept by law even if you ask us to delete it.
Our website is meant for adults, including parents and guardians requesting care for their children. We do not knowingly collect personal information online from children under 13. If you believe a child has sent us information through the website, please contact us and we will delete it.
Our website may contain links to websites or services operated by third parties. Bridgetown Dental does not control the privacy or security practices of third-party websites, and their practices may differ from ours. We encourage you to review the privacy information provided by those services before submitting personal information.
This includes services we link to for convenience, such as our Smile Club enrollment portal, payment and financing providers, and video and review sites.
We may update this policy when our practices or the law change. The “Last updated” date at the top of this policy shows when it last changed. If we make a significant change, we will post the updated policy on this page.
If you have questions about website privacy, HIPAA, or how Bridgetown Dental handles your information, please contact us at 503-286-5654 or smiles@bridgetowndental.com.
For specific information about our HIPAA privacy practices and your rights regarding protected health information, please review our Notice of Privacy Practices once the final approved document is posted.
