Adult Membership
(13 and Older) $49.00/mo. (Save at least $229.33 a year!)
- Professional dental cleanings (up to 2 annually)
- Doctor exams (up to 2 annually)

- One emergency exam per year (if needed)

For patients with regular cleanings and absence of periodontal disease.
Perio Membership
(Periodontal Disease) $99.00/mo. (Save at least $389.33 a year!)
- Perio maintenance cleanings (up to 4 annually)
- Doctor exams (up to 2 annually)
- Fluoride treatment (up to 4 annually)
- Needed digital X-rays*
- One emergency exam per year (if needed)

For patients enrolled in active periodontal maintenance due to prior treatment of periodontal (gum) disease.
